Responsibilities
- Engage members by phone, text, and email throughout their cancer-care journey.
- Identify changes in health and social needs, discuss goals and values, and support advanced care planning.
- Assess urgency, determine root causes, and establish appropriate next steps.
- Connect members with nurses, providers, healthcare and community resources, transportation, financial grants, emotional support, and insurance benefits.
- Coordinate care, including telehealth visits, diagnostic appointments, and medical treatments.
- Escalate clinical symptoms to clinical teams.
- Serve as the primary member contact while collaborating with an interdisciplinary care team.
- Work in a structured, high-volume phone queue handling inbound and outbound calls.
- Document interactions and coordinate resources while meeting productivity and quality metrics.
- Participate in coaching and development sessions and adhere to scheduled shifts, breaks, policies, and escalation pathways.
Requirements
- At least 2 years of experience in a patient-facing role involving care coordination, healthcare navigation, non-clinical case management, resource navigation, or community health work.
- Experience in fast-paced, high-volume, metrics-driven environments such as contact centers, care navigation, or care coordination.
- Ability to assess stated and unstated member needs and prioritize appropriate support.
- Ability to build rapport and trust quickly in a primarily phone-based environment.
- Empathy, strong communication, interpersonal, listening, organization, and time-management skills.
- Ability to handle confidential client information and track appointments.
- Flexibility and adaptability in response to changing member and provider needs.
- Comfort with technology and learning new systems.
- Previous remote-work experience and a quiet, secure workspace with reliable internet.

- Experience with video chat, Google Suite, Slack, or electronic health records.
Compensation and Benefits
- Two weeks of virtual onboarding and training.
- Scheduled coverage includes a 30-minute lunch and two 15-minute breaks.
