Care Coordinator

Coordinates referrals, authorizations, follow-up services, patient outreach, and care-team communication for older adults. The role requires 2–3 years of healthcare experience, EHR and medical terminology knowledge, strong organization, and HIPAA-compliant documentation skills.

Responsibilities

  • Provide administrative coordination and support for an assigned care team.
  • Coordinate daily care team huddles, including agendas, documentation, assignments, and follow-up tracking.
  • Communicate patient status changes, cancellations, no-shows, transportation barriers, access issues, and newly identified needs.
  • Monitor dashboards, worklists, and event feeds for hospital and emergency department activity, care gaps, orders, referrals, and visit completion.
  • Maintain prioritized work lists and support care-gap closure, lab orders, specimen collection, in-home test kits, durable medical equipment, and follow-up services.
  • Manage referrals from initial request through confirmed completion, including specialist and facility verification, appointments, records, authorizations, transportation, and payer follow-up.
  • Conduct telephone outreach for appointments, preventive care, care gaps, emergency-department follow-up, and missed visits.
  • Help patients understand discharge instructions and arrange necessary follow-up care.
  • Document care coordination activities, contacts, referrals, interventions, and escalations accurately in the EHR.
  • Track process metrics and maintain HIPAA-compliant patient confidentiality and data security.

Requirements

  • 2–3 years of experience in a medical office, healthcare practice, care coordination, or similar healthcare setting.
  • Experience scheduling appointments or coordinating healthcare referrals.
  • Working knowledge of medical terminology and EHR documentation.
  • Familiarity with insurance authorization, in-network verification, and prior authorization processes.
  • Understanding of HIPAA-compliant record handling and patient confidentiality.
  • Strong organization, attention to detail, proactive communication, and escalation skills.
  • Ability to engage patients and caregivers from diverse backgrounds and health-literacy levels.
  • Comfort conducting telephone outreach using cloud-based softphone technology.
  • Ability to create clear, accurate documentation.
  • Ability to work effectively in a fast-paced, team-oriented healthcare environment.

Nice-to-haves

  • CMA, CNA, EMT, or equivalent clinical certification.
  • Spanish proficiency.
  • Familiarity with Medicare, Medicaid, community resources, transportation services, or disease-management programs.
  • Experience coordinating lab orders, in-home test kits, or durable medical equipment.
  • Experience monitoring healthcare dashboards, worklists, ADT feeds, or emergency-department notifications.
  • Experience conducting post-discharge or post-emergency-department follow-up calls.

Work Location

  • Full-time, in-office position based in Memphis, TN.
  • Candidates must be located in the Memphis area and able to work onsite.
