Responsibilities
Clinical Documentation
- Accurately document patient encounters within Electronic Health Record (EHR) systems.
- Capture patient history, review of systems, physical examination findings, assessments, diagnoses, treatment plans, procedures, medications, and other clinically relevant information.
- Ensure documentation accurately reflects clinicians’ observations, clinical assessments, and medical decision-making.
- Review completed documentation for completeness, consistency, and accuracy before submission.
- Complete documentation within established turnaround-time and productivity expectations.
Clinician Support
- Provide documentation support for clinicians across specialties, including:
- Primary care
- Internal medicine
- Oncology
- Orthopedics
- Rheumatology
- Other assigned specialties
- Learn clinician-specific workflows and documentation preferences.
- Adapt documentation style and workflow to individual clinician requirements.
- Communicate professionally while supporting providers and collaborating with internal stakeholders.
Documentation Quality
- Produce documentation that meets organizational quality standards and clinician expectations.
- Apply specialty- and provider-specific documentation guidelines and best practices.
- Identify documentation inconsistencies and seek clarification through appropriate channels.
- Incorporate feedback from quality reviews, coaching sessions, and audits into daily practice.
- Participate in quality-improvement initiatives and calibration activities.
Technology and Workflow Management
- Use EHR systems and documentation platforms effectively.
- Demonstrate proficiency with ambient AI documentation technology and associated workflow tools.
- Navigate multiple systems while maintaining productivity, accuracy, and quality.
- Follow established operational workflows, standard operating procedures, and documentation processes.
- Escalate workflow, technology, or system issues through appropriate channels.
Compliance and Information Security
- Maintain compliance with HIPAA and applicable patient privacy, confidentiality, and information-security requirements.
- Protect Protected Health Information (PHI) according to organizational policies.
- Adhere to compliance, security, and documentation standards.
- Complete mandatory compliance and security training within required timelines.
Continuous Learning and Collaboration
- Complete onboarding and ongoing training programs.
- Develop knowledge of medical terminology, specialty-specific documentation, healthcare workflows, and emerging technologies.
- Stay current with documentation standards, organizational processes, and workflow enhancements.
- Collaborate with Operations, Quality, Training, Product, and other cross-functional teams.
- Participate in coaching sessions, quality calibrations, workflow reviews, and operational improvement initiatives.
- Escalate documentation, workflow, clinician, or operational concerns through appropriate channels.
- Support initiatives that improve documentation quality, clinician satisfaction, and operational efficiency.
