Senior full-stack engineer (backend-heavy) drives end-to-end projects scaling healthcare data platforms, building search/infra/LLM features using Node.js/TypeScript/AWS. Requires 6+ YOE, strong ownership, SF-based/relocate, generalist hacker mindset.

Responsibilities
- Drive projects end-to-end from ideation to production rollout: write design docs, review with team, build v0 PRs quickly for staging/pilot customers, iterate to v1 with feedback, and roll out to all customers.
- Scale services to handle 100x load, manage infra costs for savings and scalability.
- Build OpenSearch cluster for searching patient medical history.
- Transform TCP messages from 90s protocol to modern HTTP webhooks.
- Use LLMs to transform doctors' notes into structured medical data.
- Review PRs for other engineers, balancing quality and speed.
- Manage work in Linear, participate in sprint planning/retros, quarterly planning, daily stand-up, and on-call rotation.
Requirements
- 6+ years experience as a full-stack engineer, heavy lean towards back-end.
- Experience developing and maintaining cloud-based platforms, including scaling, infrastructure as code, and monitoring. Nice-to-have: Experience processing large amounts of data (moving, transforming, querying).
- Located in San Francisco or Bay Area (or willing to relocate). Nice-to-have: Familiarity with healthcare terms (FHIR, HIE, IHE, EHR/EMR, NPI, TEFCA, ADT, HL7, HEDIS, RAF, SNOMED, LOINC, ICD-10).
